Telegraphing and delamination

My Expedition had a perfect finish, the glass came out crystal clear, the epoxy cured perfectly.... but about a month later after I had already taken the kayak out I noticed a small area where the cloth apeared to have raised, showing the texture of the weave. It was a perfect boat but now it has a small blemish on the deck. Is this what they call telegraphing? And does anyone know how to prevent this from happening on my next kayak. Also, I have a strip built canoe that I built recently. I noticed when I took it out of the water the other day that it had a silver dollar sized area that had delaminated in the hull area.I didnt appear to be their when I put it in the water. I store all my boats indoors while not in use, but the building does get into the 100's in the summer. I put 5 coats of spar varnish with uv and the only time the hull is exposed to the sun is when it's upside down on the back of my truck. I don't think I hit a rock or bumped the area in any way. Could it be that I didnt clean the area well enough before laying on the first coat of epoxy? I've dripped epoxy on the cement floor on top of saw dust, dirt etc.. and it bonds so well I can't get it of with a floor scraper. I know over exposure to uv can cause delamination but I don,t think that is the case here...

Does anyone know what may have caused this so I can keep it from happening again?
