I like the job done on the table saw. Just make the setup so the blade will count in the removal of the rabbet. Try a test piece first to make sure of your measurement with the fence. I cut the deep part first then the small cross cut to finish it next.

Also I would probably go with 5/8" or 3/4" ply next time also. The form is a bit wobbly at first. The first few strips being careful to keep everything alligned while starting will stiffen up the form pretty quick. Good luck there.
