A couple of thoughts:
1) Organizations that help the handicapped probably have encountered similar problems and found solutions, although not necessarily with kayak paddles.
2) I don't think you need fingers on the paddle shaft during the push phase of the stroke. I don't use mine.
3) I'm a little leery of attaching the paddle to the paddler with velcro or straps. I can see some safety issues.
4) What about some sort of split shaft arrangements? Make a slot in a triple width paddle shaft about one palm thickness. During the pull phase of the stroke the paddler rotates their hand or makes a fist to lock their hand in the slot. You might have to be careful with blister formation with this one.
