Have you considered looking at the device that high-bar gymnasts use to help hold them onto the bar? It appears to be a padded "J" shaped metal plate that attaches around their wrist. The hook on the "J" captures the bar in the same way that it could capture the handle of the paddle. In your description of your friend, it sounds like they could use the thumb and pinkie to help hold the paddle into the "J" to maintain contact and control. You would have to get one to try out and determine if it is suitable, or if it can be easily modified.
Just a thought.
