Extensions come in two flavors: cast iron and sheet metal. Find old cast ones if you can. My experience with even heavy sheet metal extensions is that they sag a little and need shimming. A homemade plywood or Formica-covered arrangement could be made. Use angle iron where it meets the saw and legs outboard to keep it from drooping.
Some shops build a table all around the saw for extensions in all directions. This is especially good for working with sheet goods.
Don't worry too much about hole spacing if you find extensions. Just drill new holes.
