It's simply two overhand knots tied back-to-back around the standing cord.

A double fisherman's knot is the same thing, except that you pass the end around the standing cord twice instead of once, before slipping it underneath. It's much more secure, but bulkier. I use it frequently when connecting ropes for rappelling off ice climbs.
