I haven't seen it happen. I have cleats fore and aft, and do cowboy scramble rescues myself (rear deck) and assisted rescues of others (foredeck).
Not to say it still isn't a risk. I haven't seen the situation Lee described, but it could happen.
For a strictly paddle park, yours is much less obtrusive, and probably safer. I wasn't sure if maybe I'd like to try my hand at kayak sailing when I installed mine. My cleats also act as the inward anchor points for my running decklines.
Hard padeye: never snagged, but I did catch a cuticle on one once and it bled nicely. When your hands are already wet, it looks like you're really bleeding. I also had one busted off in a vigorous game of Kayak Ultimate Frisbee (right before I saw you in Port Townsend in October). I think it was the same padeye. Serves it right.
