Hi Malcolm,
The short answer would be "stupidity". The longer version is that I guide sea kayaking trips in Chilean Tierra del Fuego, for Wilderness Travel. I was climbing from the tender onto the support boat when it dropped into the water of Bahia Mansa on the Straits of Magellan. I estimate it was about 40 minutes by the time we got a diver and he suited up and recovered it. (see link to photo) One of my favorite quotes is "Adventure, rightly stated is only incompetence" This incident fit that description.
