Hi Tim, it's basically everything you see in the picture... a board with a hole in it. I clamped it to two sandbagged sawhorses, setup some feather boards and went to town. I layed out the holes by making a template out of posterboard. The screws go directly into the threads already in the bottom of the router. There are small nuts on the other side (hidden) that help hold everything tight. It's a hassle to adjust but it does hold the tool reliably.

For zero clearance, you would start with a solid fence and feed it laterally over the spinning cutter. That would leave the smallest possible opening; nice but not required.
