I've used a vice-grip fastened to a stick to successfully put nuts on bolts in both the far stern and bow areas several times. I use this for anything with a strong vertical pull.
Altho I like the idea of T-nuts with careful tightening.
I have also used stainless screws for padeyes or flat webbing loops (which I prefer) screwed into the end pour. The tension on a padeye/loop for safety lines, bungees, etc. is usually horizontal and perpendicular to the long axis of the screw rather than vertical and pulling directly up on the screw head. If this is the case, you don't need a really strong bolt set up.
