Looks like when you were trimming the glass you were using a sawing stroke, cutting on both the downwards as well as the upwards strokes. When trimming glass over wood you can only cut on the downward stroke so as not to lift the glass from the edge. Also helps alot if you do this at the green stage of the epoxy, just as it's turned to a rubbery state. Have to have lot of extra razor blades though as the gummy epoxy makes them sticky pretty quickly.

Can be sanded back to where it's firmly joined then patched with new glass. Shouldn't be able to see the patch if you're careful.
