I built a strongback exactly as you did (but I wouldn't make another out of MDF - too flexible) and I just opened up the holes in the forms as Kudzu suggested. You have to align the forms to each other with a laser or strings or whatever so you can't go by the position of the 2x4 holes printed on the forms anyway.
