Recently if I preview a reply, the optional image URL window is filled in with the details of the last image I posted. So then if I post, that image is attached.
What makes it worse is that it isn't even my photo. I was helping someone else get his photo posted.

The URL window is empty when I start typing, but fills itself in when I do a preview.
