Solvent wipes (and tack cloths) have been identified as frequent causes of fisheyes between epoxy coats. Refineries frequently use recycled materials to remanufacture solvents that may introduce trace amounts of materials (including oils) in the solvents. Contaminated solvent wipes between the polyurethane coats could be your problem. Try eliminating the solvent wipe and use just soap and water with a running water (hose) rinse.
