None of the PFDs we wear are designed to keep the head upright, those are just too bulky. If hypothermia knocks us out, we go face forward and drown. Our PFDs work only if we are conscious.
(I don't know if there are any supplemental inflatables that we could inflate (CO2 cylinder) if we get to the point where we are beyond getting back in our kayak, or swimming to shore and all we can do is wait.)
More horror stories.
A friend spoke 6 languages, ran a business, hit his head in an accident, can no longer speak, work. Makes me want to wear a helmet just walking down the street. (I park in a lot behind my school where about 10-15 years ago another teacher getting in her car was hit on the head by a thrown rock--coma and death.)
My girlfriend just told me her sister and a friend were swimming in a pool. They were just by themselves, but the friend drowned before the sister even knew something was wrong.

I recommend (actually I think I've harped on it) reading Laurence Gonzales's book, "Deep Survival: Who Lives, Who Dies and Why." It covers a lot of the stuff many of us see as 'stupid' after the tragedy.
One thing all humans, myself included, are likely to do is 1. We do something risky, and we survive so we 2. think it's not as dangerous as it really is. If we do this several times, we're just upping the ante.
The most fun part of the book was about getting lost. I use this whenever I'm in the fog. It's amazing how quickly the panic of feeling lost sets in--just as soon as we realize what we are looking at doesn't fit the mental map of where we think we are.
