Looks like the PDF is full size (sheets are 24x36), so from that the forms could be made, and it includes the templates for the panels as well.

He does mention fitting the forms to get the shape right, not having them at a specific location in the description...

I didn't look closely at the tables of offsets, as it looked like one was for creating the panels, and the other was for the shape of the forms. Both of which can be made from the templates in the plans.
