No, they don't need padding. They are comfortable because they move with you as you rotate your torso.

They are more convenient because they give you support higher up your back when you sit up and they swing down so that they don't get in the way when you want to do a roll.

They look a bit like this. There are webbing straps going forward to the cheek plates, a cord to the top of the backrest and a short loop of bungee on the bottom. I don't have a good photo that shows how they are rigged, but the explanation is all there in Nick's book.
