In other words, it's not always obvious whether the designer or builder specifically chose to do something in some certain way for a specific reason, or if he simply happened to do it that way as a consequence of other design decisions, and if he'd made different design decisions then he may well have done it some other way.

I can say that most design decisions are made for a specific reason. Mostly they are made to make the design a good design (by the designer's definition), and usually the process of building is secondary. If the design idea is good enough they will find a way to make it. That said, different building methods may suggest design ideas so there is a feed-back loop.
